UESP requests that each school needs to fill out this form that includes their schools winners of the (2) UESP Savings Certificates. If you as a school need your certificates before the April forum meetings, please contact Jennifer and we will send you the certificates. Fax (801-321-7299) or mail to (UESP, Attn: Christi Scow, PO Box 145100, Salt Lake City, UT 84114-5100) the filled out form back to UESP by June 30, 2012. |
